• Proceed to the Training Office of DMC and verify the schedule for training
• Ask for the Schedule of entrance exam
• Have your name listed with them and choose your area
• Pay the Exam fee per module at the cashier
• Go back to the training office and show them your receipt so that you will be booked.
• They will advise you for the date of your examination.

If you pass their exams, below are the documents that you need to prepare

• Letter of Application ( Indicating Nurse Trainee Position )
  
• Comprehensive Resume with 2x2 picture
  

Photocopy of the following

• Transcript of Records
• College diploma
• Board rating
• Board Certificate
• PRC License
• PNA membership card
• DNA ( Davao Nurses Association ) Membership card
• Medical Certificate with laboratory results ( u/a, f/a, cbc, cxr, drug test) *
  
• Police clearance
• Hepa B Vaccination card
• 1 2x2 pic
• Training agreement (ask for this at the training office)
  

* Should be done at DMC Laboratory only
