If your seeking for job abroad, have all your documents be Certified,Authenticated and Verified by the CHED (redribbon)

Here the list of documents that you need to prepare:

- 2 x 2 colored picture
- Photocopy of the following documents:
- Diploma
- Transcript of Records
- Summary of Related Learning Experience
- Recommendation Letters from the School you graduated
- Good Moral Certification
- PNA Membership
- Board Rating and Certificate on Process of License ( You can get this at PRC-Davao for only
P75.00/ certificate)
- SSS Membership. Get E1 form if you don’t have your Social Security Number yet. Bring with
you your ORIGINAL NSO Authenticated Birth Certificate. No fees to
be collected ( Go to SSS Bajada )- Tax Identification Number ( go to BIR –R11 at Torres st
fronting Assumption Church )
- NBI / Police Clearance ( For NBI go first to NBI office infront of Victoria Plaza and you will be
asked to fill out some personal information, bring with you any valid ID, then proceed to NBI
2nd flr Victoria Plaza for your picture. Payment 115.00 only. While for your police clearance
you can get it to any Police station near you)- Philhealth Membership (Go to Philhealth office
at Quimpo Blvrd, bring with you your NSO Birth Cert)
