Photocopy of the Following:

· Updated Curriculum Vitae / Resume
· Current PRC ID
· Board rating and Board Certificate
· Transcript of Records with RLE
· BSN Diploma
· Good moral certification
· NBI Clearance (current)
· Birth Certificate (NSO)
· 2 pieces 2x2 colored pictures (white background)

Training Fee: 10,000.00 (3months training program)

- Present ORIGINAL documents
- INCOMPLETE requirements will not be accepted
- Submission of requirements will be entertained on Monday-Friday 9am-12nn at the Professional Development and Training Department 9th flr Mezzanine, MMC

APPLICATION PROCEDURE
- Submit complete requirement photocopy only to Professional Development and Training Office Department office for screening
- Wait for Call or Text by the Nursing Service for interview
- Come for panel interview and pass it
- Bring ORIGINAL documents
- Obtain medical clearance with Xray
- Pay 10, 000 at Level 5
- Attend CETN and present registration form and receipt
