Philippines )

- Current BLS Card ( Philippine National Red Cross )

- Current PRC License ID

- Current PNA Membership Card

- HBsAg Screening *

- Chest X-Ray*

- 1 pc 1x1 and 1 pc 2x2 Pictures

* HBsAg Screening and Xray should be taken at NKTI Laboratory and should not be more than 6 mos to date.

Note: For Dialysis Management Course, you should have a Certificate of Employment of at least 6 months in a 150 bed hospital.
